Traditionally men visit the doctor less frequently so there are fewer opportunities to review family history, lifestyle and other factors that may identify potential health issues that can be managed proactively. Chronic Disease Management
At Highlands GP we want to work with patients to manage chronic diseases to assist patients to be able to live independently and actively.
Conditions such as COPD, hypertension, osteoporosis, renal conditions, asthma, arthritis and chronic pain all require regular review and management and eligible chronic disease management assessments and plans will be bulk billed.
An annual Health Assessment is offered to patients who are over 75 and/or have a chronic health condition. Eligible patients will be bulk-billed for the Health Assessment and associated care plans.
Health Assessments are designed to review a persons overall health and wellbeing. It also helps to identify any specialist care and/or allied health services such as podiatry, physiotherapy, occupational therapy, etc patients could benefit from to support independent living.
Highlands General Practice can arrange for you to have a Health Assessments either at the practice or one of our nursing team can visit you at home.

We also work with community services to help our patients to access services to help support independent living such as home care, meals on wheels, transport services, mental health services, etc.
